Description
Posteriorly, the arachnoid mater is rendered as the translucent leptomeningeal sheath draped over the occipital lobes, cerebellar hemispheres, and dorsal aspect of the brainstem, continuing caudally around the upper cervical spinal cord. Sulci are bridged by arachnoid trabeculae spanning to the pia mater, so the membrane follows the gross contour of the cerebrum while tenting over deeper grooves. Superficial veins course within the subarachnoid space and converge toward dural venous sinuses along the midline, while vascular channels and nerve elements track inferiorly toward the foramen magnum. Clear relationships. This posterior orientation is the teaching angle that best clarifies where cerebrospinal fluid circulates around the cerebellomedullary cistern and dorsal cervical cord, and why blood in the subarachnoid space layers dependently over the posterior fossa on imaging. It also supports surgical and procedural planning: suboccipital craniectomy and posterior fossa approaches require arachnoid opening to access the cerebellomedullary fissure, and failure to respect arachnoid planes can tether cranial nerves and vessels. Arachnoid granulations and the venous drainage pathway provide the clinical link to communicating hydrocephalus and impaired CSF resorption after meningitis or subarachnoid hemorrhage.
