Removed calvaria exposes the cerebral hemispheres with the pia mater (leptomeninx) draped tightly over the gyri and dipping into the sulci, in direct apposition to the cortical surface. Fine pial arteries and veins course within the subarachnoid space along the brain’s contours, then turn into the sulcal depths toward the cortex. The pia bridges neither across sulci nor over the longitudinal fissure; it follows each convolution while the falx cerebri lies more medial and superior between the hemispheres. Pial anatomy matters because these small surface vessels represent the last extracerebral segment before penetrating arterioles enter the brain, and their course predicts where subarachnoid blood tracks in aneurysmal rupture. Neurosurgical approaches that open the sylvian fissure or work along the convexity (for tumor resection, cortical mapping, or epilepsy surgery) rely on preserving pial planes and avoiding pial vessel injury that can convert a clean arachnoid dissection into cortical ischemia. Venous anatomy, too, lives on this surface. Tearing a bridging vein near the superior sagittal sinus is a classic setup for subdural hematoma.
