Description
Running inferiorly from the diaphragmatic hiatus, the abdominal aorta descends in the midline anterior to the vertebral column, framed superiorly by the lower ribs and inferiorly by the pelvic ring. Paired renal arteries course laterally toward the kidneys, while the celiac trunk arises just below the diaphragm and divides toward the foregut, with the superior mesenteric artery emerging slightly inferior to supply the midgut. Lower in the abdomen, the inferior mesenteric artery angles toward the left colon, and the aorta terminates at the level of the fourth lumbar vertebra by bifurcating into the right and left common iliac arteries, which continue along the pelvic brim. Anterior orientation matters for teaching and for operative planning because the major visceral branches arise from the aorta in a predictable cranio-caudal sequence that surgeons and interventionalists use to localize bleeding, ischemia, or aneurysmal disease. Mesenteric ischemia often centers on embolic or thrombotic compromise of the superior mesenteric artery, while abdominal aortic aneurysm typically involves the infrarenal segment just proximal to the iliac bifurcation, a relationship that becomes easier to grasp when the bony landmarks of the rib cage and pelvis are kept in view.
