Description
Arising from the left lower lobar bronchus, the collective basal segmental bronchus courses inferolaterally within the lower lobe and then begins to divide toward the basal segments. From a left lateral perspective, the bronchial trunk sits posterior to the expected position of the left inferior pulmonary vein and inferior to the hilum, with its first branches angling anteriorly and posteriorly toward the diaphragmatic surface. Segmental rami that will supply the anteromedial, lateral, and posterior basal segments radiate distally, following the typical bronchovascular pairing pattern within the bronchopulmonary segments. Understanding the basal segmental bronchus matters when you need to localize disease within the left lower lobe and speak in segmental terms that map to bronchoscopy, CT reporting, and segmentectomy planning.
