Description
Seen from a posterior abdominal perspective in an adult male, the pancreas is centered against the posterior abdominal wall with its head nestled in the C loop of the duodenum on the right and its tail extending leftward toward the splenic hilum. Along the superior border, the tortuous splenic artery tracks toward the tail while the splenic vein runs posterior to the gland, receiving small pancreatic veins before joining the superior mesenteric vein to form the portal vein. Inferior to the neck and uncinate process, the superior mesenteric artery and vein pass anterior to the third part of the duodenum and posterior to the pancreas, giving rise to pancreaticoduodenal arcades that link celiac trunk and SMA territories. A dense mesenteric vascular fan overlays the small intestine, with the inferior mesenteric vessels oriented more caudally toward the left colon. Posterior orientation matters because it clarifies relationships that are easy to lose in anterior teaching views, especially the retropancreatic course of the splenic vein and the proximity of the SMA/SMV to the uncinate process. This is the anatomy behind common operative decisions: pancreatic head carcinoma often abuts or encases the SMV-portal confluence, and pancreatitis or pseudocyst formation can erode the splenic artery or gastroduodenal artery, creating life-threatening hemorrhage. Portal hypertension and splenic vein thrombosis also make more sense when you can trace venous drainage from the pancreas into the portal system. Surgical educators will find this useful for Whipple procedure planning discussions, for explaining arterial variants relevant to pancreaticoduodenectomy and distal pancreatectomy, and for correlating CT angiography landmarks in upper abdominal radiology teaching.
