Description
Running along the anterior compartment of the arm, the brachial artery descends from the distal border of teres major and tracks inferiorly in the medial bicipital groove toward the cubital fossa. Proximally it lies medial to the humerus and quickly becomes accompanied by the median nerve, which crosses from lateral to medial over the vessel as it approaches the elbow. Deep and posteromedial to the biceps brachii, the artery gives off the profunda brachii artery to the posterior compartment while muscular branches supply brachialis and biceps. Small arterioles can be appreciated coursing distally into the surrounding soft tissues. Palpation and compression of the brachial artery just medial to the biceps tendon forms the basis of routine blood pressure measurement, and this anterior perspective clarifies why improper cuff placement or arm position can distort auscultatory findings at the cubital fossa. The relationship to the median nerve matters in supracondylar humeral fractures and iatrogenic injury, where swelling or malreduction can compromise flow or produce nerve ischemia. Clear separation of the profunda brachii also supports teaching of collateral circulation around the elbow, a practical point when proximal arterial injury or ligation is considered.
