Description
Seen from a medial perspective, the paired brachial veins (venae comitantes) track proximally along the medial aspect of the arm, closely flanking the brachial artery within the medial bicipital groove. Superficial to this neurovascular bundle, the biceps brachii occupies the anterior compartment while portions of the triceps brachii remain posterior, with the deltoid forming the rounded contour of the shoulder superiorly. Proximally, the veins converge toward the inferior border of teres major where they continue as the axillary vein, with the surgical neck of the humerus and adjacent scapular elements providing bony orientation. Nerve elements of the brachial plexus accompany the vessels in the arm. A medial angle on the brachial veins matters because this is the corridor where venous outflow from the forearm and arm is organized into deep companion veins, and where iatrogenic and traumatic problems often declare themselves. Venipuncture, dialysis access planning, and upper extremity deep venous thrombosis workups benefit from a clear distinction between deep brachial veins and the more superficial basilic and cephalic pathways. It also supports teaching the relationship between the median nerve and brachial artery at the elbow and mid-arm, a frequent landmark during brachial artery exposure and repair.
