Description
Anteriorly oriented, the distal humerus is centered on the humeral condyle, with the capitulum positioned lateral to the trochlea and separated by the shallow capitulotrochlear groove. Medial and lateral epicondyles bracket the articular surface, projecting proximally from the condylar mass and defining the medial and lateral columns of the distal humerus. Superior to the joint line, the distal metaphysis flares as it transitions to the humeral shaft. Clear landmarks throughout. Clinical teaching often collapses the “condyle” into a single unit, but the anterior view forces the distinction that matters for stability and motion: the trochlea guides the ulnohumeral hinge while the capitulum pairs with the radial head during flexion and forearm rotation. Orthopedic reduction of supracondylar fractures, and fixation planning for distal humerus fractures, depends on reading these contours and using the epicondyles as reliable palpation and fluoroscopic reference points when the joint surface is obscured by swelling. Lateral elbow pain syndromes also map here, because the common extensor origin on the lateral epicondyle sits just proximal to the radiocapitellar articulation implicated in lateral epicondylitis and posterolateral rotatory instability.
