Description
Anteriorly, the humeral head appears as a smooth, spherical articular surface capped by hyaline cartilage and seated against the shallow glenoid cavity of the scapula. Superior and lateral to the joint line, the clavicle trends toward the acromion, while the anterior rim of the glenoid and the scapular neck define the socket’s margins. Across the front of the capsule, the superior, middle, and inferior glenohumeral ligaments can be read as bands spanning from the glenoid and labral region toward the anatomical neck of the humerus, with the proximal humeral shaft continuing inferiorly. Portions of the anterior thoracic cage sit medial to the joint, grounding the shoulder in the chest wall. This anterior perspective matters because it aligns with the operative and exam-facing surface of the glenohumeral joint. Anterior shoulder dislocation, by far the most common direction, typically drives the humeral head anteroinferiorly relative to the glenoid, stressing the anterior-inferior capsule and inferior glenohumeral ligament complex and predisposing to a Bankart lesion at the anteroinferior labrum. A clear read on these relationships helps when teaching why external rotation and abduction can reproduce apprehension, and why stabilization procedures focus on restoring the labral bumper and capsuloligamentous tension. Orientation is everything. Use this illustration in musculoskeletal anatomy and orthopaedics teaching to pair surface anatomy with joint mechanics, or in a surgical atlas section describing anterior approaches to the shoulder and capsulolabral repair.
