Palmar venous drainage is centered on the deep palmar venous arch, a transverse channel lying deep to the palmar aponeurosis and flexor tendons and closely applied to the level of the carpal bones and proximal metacarpals. From an anterior (palmar) perspective, paired deep palmar digital veins converge into palmar metacarpal veins that feed the arch, which tracks across the palm with the thenar region laterally and the hypothenar region medially. Proximally, venous outflow continues toward the wrist through deep veins accompanying the radial and ulnar arteries (venae comitantes). Relationships matter. Because the deep palmar venous arch sits adjacent to the deep palmar arterial arch and the palmar interossei, it is a structure you account for when teaching layered palmar anatomy and when planning dissection or surgical exposure in the distal palm. Lacerations over the distal palmar crease and carpal tunnel level incisions can encounter deep venous channels that bleed briskly and obscure the field, and postoperative hematoma in this compartment can track along synovial sheaths. Venous anatomy also frames how infection spreads in the hand, with deep space infections (midpalmar or thenar) and flexor tenosynovitis affecting venous return and swelling patterns.
