Descending Genicular Artery, Anterior View

Description

Arising from the femoral artery in the distal thigh, the descending genicular artery is shown on the anteromedial aspect of the knee just proximal to the medial femoral epicondyle and the adductor hiatus. From its short trunk, the vessel divides into an articular branch that courses toward the medial femoral condyle and patellar plexus, and a saphenous branch that runs distally with the saphenous nerve along the medial side of the joint toward the proximal tibia. Deep to the patella, the femoral trochlea and the anterior tibial plateau frame the joint space, with menisci and articular cartilage indicated between femur and tibia. Companion veins track alongside the arterial channels. Anterior visualization of the descending genicular artery matters because it is a key contributor to the periarticular genicular anastomosis that maintains perfusion around the knee when the femoral artery is flexed or temporarily occluded. Surgeons encounter this vessel during medial approaches and during harvest of a saphenous or descending genicular artery perforator flap, where injury to the saphenous branch can compromise medial skin perfusion and provoke painful neuroma or sensory loss along the medial leg. It also provides a practical landmark for teaching the relationship between the adductor canal contents and the medial knee vascular network. Short, teachable anatomy.
