Description
Arising from the femoral artery in the distal thigh, the descending genicular artery courses inferiorly and slightly medially toward the posterior and medial aspect of the knee, where it approaches the adductor hiatus and the popliteal fossa. From a posterior perspective, the vessel is seen tracking deep to the sartorius and along the medial femoral condyle, with branches that descend toward the tibial condyle and the posterior capsule. Nearby lie the distal femur and proximal tibia with their condylar articular surfaces, and the fibula appears lateral to the tibia as a consistent landmark. Accompanying veins parallel the arterial pathway and converge toward the popliteal venous drainage. Posterior knee vascular anatomy matters when you need to understand collateral circulation around an often-stenotic segment, and the descending genicular artery is a key contributor to the genicular anastomosis with the superior medial genicular, inferior medial genicular, and recurrent branches. Occlusion of the superficial femoral artery proximal to the adductor hiatus can shift flow demands onto this network, preserving distal perfusion when the popliteal segment is compromised. Surgeons also care about this vessel during medial approaches to the distal femur and knee, as well as during harvesting of medial femoral condyle osteocutaneous flaps, where the descending genicular system (including its saphenous branch) may form the pedicle. Small vessel, big consequences.
