Description
Running along the visceral surface of the right hepatic lobe, the fossa for the gallbladder (fossa vesicae biliaris) forms a shallow, elongated bed between hepatic segments IVb and V. The fundus of the gallbladder projects inferiorly beyond the inferior border of the liver, while the body lies in close apposition to the hepatic parenchyma and the neck turns posteromedially toward the porta hepatis. Medially, the fossa approaches the fissure for the ligamentum teres, and posteriorly it aligns with the plane leading toward the inferior vena cava groove. A defined peritoneal reflection typically leaves part of the gallbladder adherent to the liver without a complete serosal covering. Anatomically, this is the working map for cholecystectomy. The gallbladder bed is where hepatic bleeding and bile leakage can declare themselves after difficult dissection, and appreciating its relationship to the adjacent hepatic segments helps explain why inflammation can tether the gallbladder to liver and distort landmarks. In acute cholecystitis or chronic fibrotic change, the fundus-first approach often begins by freeing the gallbladder from this fossa and proceeds toward the neck where Calot’s triangle structures become the next hazard zone. Small subvesical bile ducts (ducts of Luschka) may traverse the bed. They matter.
