Oriented from an inferior viewpoint, the visceral (inferior) surface of the adult male liver is presented with emphasis on the fossa for the gallbladder (fossa vesicae biliaris). The gallbladder bed lies to the right of the fissure for the ligamentum teres and immediately inferior to the porta hepatis, forming a shallow indentation between the right lobe laterally and the quadrate lobe medially. Expect the hepatic parenchyma to wrap around this fossa, with the cystic impression positioned anterior to the groove for the inferior vena cava and medial to the more posterolateral renal and colic impressions. Clean landmarks. Surgical orientation often depends on recognizing the gallbladder fossa before Calot’s triangle is dissected, since the cystic plate and adjacent hepatic tissue define the plane for safe separation of the gallbladder from the liver. In acute cholecystitis, edema and fibrosis in the gallbladder bed can tether the fundus and body to the hepatic capsule, increasing bleeding risk during laparoscopic cholecystectomy and making the relationship to the porta hepatis clinically unforgiving. This inferior perspective also supports teaching the segmental neighborhood: the fossa typically overlies the junction of hepatic segments IV and V, a practical reference when correlating operative findings with CT or MR reporting.
