Description
Running along the dorsum of the male forefoot, four bipennate dorsal interossei occupy the intermetatarsal spaces between the shafts of the first through fifth metatarsals. Each muscle belly sits deep to the dorsal soft tissues and is oriented longitudinally, with fibers converging distally toward the metatarsophalangeal joints and the bases of the proximal phalanges. Laterally and medially, the interossei are framed by the metatarsal cortices and the intermetatarsal ligaments, while the extensor tendons course more superficially toward the toes. Small muscles. Big impact. Dorsal interossei are the primary abductors of digits 2 to 4 (DAB), and their distal insertions blend with the extensor expansions to help balance extension at the metatarsophalangeal joints against flexion at the interphalangeal joints during stance and push-off. Intrinsic muscle weakness, as seen with lateral plantar nerve neuropathy, diabetic neuropathy, or cavovarus patterns in Charcot-Marie-Tooth disease, shifts that balance and contributes to claw toe and metatarsalgia.
