Description
Running in the anterior compartment of the leg, the extensor digitorum longus (EDL) lies lateral to tibialis anterior and medial to fibularis (peroneus) longus and brevis, with extensor hallucis longus typically interposed between it and tibialis anterior distally. Its muscle belly arises from the lateral tibial condyle and proximal fibula, then narrows into tendons that pass deep to the superior and inferior extensor retinacula at the anterior ankle. Distal to the retinacula, the EDL tendons spread on the dorsum of the foot to the lateral four toes (digits 2 to 5), forming dorsal digital expansions to the middle and distal phalanges while remaining superficial to the tarsal bones and metatarsals. Clear topography. That anterior trajectory matters clinically because the EDL shares a tight fascial compartment with tibialis anterior, extensor hallucis longus, and the deep fibular nerve, so rising compartment pressures after tibial fracture or reperfusion can produce painful weakness of toe extension and sensory change in the first dorsal web space. The same anatomy explains foot drop patterns in common fibular nerve injury at the fibular neck, where loss of EDL function compounds loss of dorsiflexion and contributes to a high-stepping gait. At the ankle, its tendons serve as landmarks in anterior arthroscopy portals and in tendon transfer planning for chronic peroneal nerve palsy. Educators will reach for this anterior female figure when teaching lower-limb compartment anatomy, surface relationships at the anterior ankle, and the mechanics of toe extension during gait in gross anatomy and kinesiology courses.
