Description
Running along the lateral compartment of the leg, the fibularis (peroneus) longus is emphasized from an anterior perspective on a female body, with its muscle belly lying superficial to the proximal and middle fibula and lateral to the tibialis anterior. Inferiorly, its tendon courses posterior to the lateral malleolus at the ankle, adjacent to the fibularis brevis tendon, before turning plantarward toward its insertion on the base of the first metatarsal and medial cuneiform. Medially, the tibia and anterior crest form the landmark border that separates this lateral muscle mass from the extensor group. Fibularis longus matters because it is a primary evertor of the foot and a dynamic stabilizer of the transverse arch, so overuse and poor mechanics often present as lateral ankle pain with peroneal tendinopathy or tenosynovitis. After an inversion ankle sprain, the peroneal tendons can sublux or tear behind the lateral malleolus, and the clinician has to distinguish that from lateral ligament injury and from superficial fibular (peroneal) nerve irritation as it exits the lateral compartment.
