Description
Centered in the pelvis, the uterus sits in the midline with the cervix projecting inferiorly toward the vaginal canal, while the uterine (fallopian) tubes course laterally along the superior uterine margins to reach the ovaries at their distal ends. Each tube terminates in fimbriated infundibulum draped near the ovarian surface. On one side, the ovary appears enlarged and ovoid, with a smooth, tense capsule and multiple subcortical cystic follicles giving a peppered or string-of-pearls external pattern; the contralateral ovary provides a quieter reference with fewer visible follicular profiles. Spatially, the ovaries lie lateral to the uterine fundus, and the fimbriae sit posterolateral to the uterine cornua as they approach the ovarian poles. Gross ovarian morphology matters in polycystic ovary syndrome (PCOS) because the external appearance often tracks with the underlying thickened tunica albuginea and hyperplastic stroma that contribute to chronic anovulation. Seeing the enlarged ovary adjacent to otherwise typical uterine and tubal anatomy helps distinguish isolated ovarian pathology from tubal factor infertility or uterine structural causes of abnormal uterine bleeding. Surgeons and trainees also recognize this surface pattern during laparoscopy, where an enlarged, pale ovary with multiple cortical cysts may prompt discussion of ovarian drilling, while still weighing the risks of adhesions and diminished ovarian reserve.
