Description
Centered in the female pelvis, the ovary is shown as an ovoid gonad with a cystic mass expanding the cortical surface and distorting the normal contour. Along the periphery, the tunica albuginea and surface (germinal) epithelium would be stretched over the lesion, while residual ovarian cortex containing follicles is compressed toward the poles and along the margin. Deeper, the medulla with its hilar vessels lies medial, and the cyst projects laterally toward where the infundibulopelvic ligament and fimbriae of the uterine tube approach the ovarian surface. Scale matters. Ovarian cysts are a daily diagnostic problem because benign functional cysts, endometriomas, and cystic neoplasms can all present as a unilateral adnexal mass, yet management hinges on morphology, size, and the likelihood of torsion or malignancy. The outward bulge and apparent tension on the capsule mirror the clinical scenario in which a cyst increases ovarian volume, predisposing the adnexa to twist around the suspensory ligament, compromising venous outflow first and then arterial inflow. This external perspective also supports teaching the difference between a simple cyst that typically preserves a thin, smooth capsule and complex processes that may thicken the wall, form adhesions, or obscure normal ovarian landmarks during laparoscopic cystectomy.
