Description
Prominent in the anterior neck, the external jugular vein courses inferiorly and superficially, crossing obliquely over the sternocleidomastoid muscle before turning toward the root of the neck. Superiorly, the vessel is shown in relation to the angle of the mandible and parotid region where it typically forms from the posterior auricular vein and the posterior division of the retromandibular vein. Medial to it, the internal jugular vein lies deeper within the carotid sheath, separated from the skin by the platysma and investing cervical fascia. Small superficial venules converge along the lateral neck to join the external jugular vein. Easy to miss. This anterior perspective matters because the external jugular vein is a surface landmark used for bedside assessment of venous distension and central venous pressure, but its behavior differs from the internal jugular vein because of valves and its superficial fascial course. The vein’s crossing of the sternocleidomastoid also explains why it is vulnerable to laceration in penetrating trauma and why air embolism is a real concern when the open vessel is held patent by the deep cervical fascia. For procedural teaching, the image clarifies why external jugular venous cannulation can be deceptively straightforward yet prone to failure as the vein dips deep to enter the subclavian vein near the clavicle.
