Description

Medial aspect of the fibula is oriented toward the tibia, so the sharp margo interosseus (interosseous border) runs longitudinally along the shaft and defines the attachment line for the interosseous membrane of the leg. Superiorly, the caput fibulae and collum fibulae sit lateral and slightly posterior to the proximal tibia, with the facies articularis capitis fibulae for the proximal tibiofibular joint turned medially. Distally, the shaft broadens into the lateral malleolus, whose medial surface contributes to the talocrural mortise while remaining lateral to the distal tibia. Clear bony geometry. A medial view matters because it isolates the tibia facing surface of the fibula, the side surgeons and anatomists reference when describing syndesmotic integrity and the distal tibiofibular relationship. High ankle sprains involve the anterior inferior tibiofibular ligament and interosseous ligament, and appreciating the fibula’s interosseous border helps explain how force can propagate proximally with external rotation injuries. Proximally, the fibular head is a key landmark for the common fibular (peroneal) nerve as it winds around the neck, a frequent site of neuropraxia after trauma or prolonged compression.
