Description
Positioned in lateral profile, the fibula (calf bone) runs parallel to the tibia along the lateral side of the shank, tapering distally into the lateral malleolus at the ankle. Superiorly, the fibular head and neck sit posterolateral to the proximal tibia near the knee, with the shaft descending inferomedially toward the ankle mortise. Along the shaft, the interosseous border faces medially toward the tibia, while the lateral surface remains subcutaneous for much of its length. A slender bone. But an unforgiving landmark. Orthopedic exams and ankle radiographs often hinge on clear appreciation of the lateral malleolus, because its distal position relative to the medial malleolus defines the talocrural joint’s lateral buttress and helps assess syndesmotic integrity. Fracture patterns that involve the distal fibula, such as Weber B and Weber C injuries, correlate with the level of fibular fracture relative to the syndesmosis and influence fixation strategy; the lateral view is also the one you want when teaching how the fibula rotates and translates in external rotation ankle injuries. Proximally, the fibular neck is the hazard zone for the common fibular (peroneal) nerve, a point worth flagging when discussing casts, fibular head fractures, or surgical approaches around the lateral knee.
