Description
Arising from a posterior perspective, the fifth lumbar vertebra (L5) is presented in continuity with the superior lumbar segments and the sacrum, with the coccyx aligned inferiorly in the midline. The spinous process sits centrally, projecting posteriorly from the junction of the paired laminae, while the pedicles run anterolaterally toward the vertebral body and frame the vertebral arch. Laterally, the transverse processes extend from the vertebral arch, and the superior and inferior articular processes define the zygapophysial (facet) joints, adjacent to the intervertebral foramen region where the L5 spinal nerve exits between L5 and S1. L5 warrants its own plate because it is the transitional vertebra between the mobile lumbar spine and the fused sacrum, and its posterior elements anchor key clinical landmarks. Facet orientation at L5 to S1 influences lumbar stability, and defects through the pars interarticularis (spondylolysis) most often involve L5, predisposing to anterolisthesis at the lumbosacral junction. This is where radicular pain begins. Educators can drop this posterior L5 view straight into gross anatomy and musculoskeletal modules to teach vertebral arch anatomy, facet joint relationships, and the boundaries of the intervertebral foramen.
