Description
Centered between adjacent lumbar vertebral bodies, the intervertebral disc is presented in cutaway to expose the annulus fibrosus as concentric lamellae of fibrocartilage surrounding a paler nucleus pulposus. Superior and inferior to the disc, the vertebral endplates form the immediate interface with cancellous bone within the vertebral bodies. Posterior elements are included for orientation, with the spinous process projecting posteriorly and the transverse processes extending laterally from the vertebral arch. Lamellar architecture is the story here. A sagittal-style section through a lumbar motion segment gives you the clearest teaching view of why disc mechanics fail in predictable ways. Posterolateral annular fissures allow nucleus material to migrate toward the vertebral canal, a common pattern behind lumbar disc herniation with L5 or S1 radiculopathy, while superior or inferior endplate weakness predisposes to Schmorl nodes and adjacent marrow signal change. Disc degeneration also begins as a biochemical shift in the nucleus pulposus with loss of hydration, which this view helps connect to narrowing of disc height and increased facet loading even before gross osteophytes dominate the picture.
