Description
Running on the anterior surface of the psoas major, the genitofemoral nerve (L1 to L2) emerges from the lumbar plexus and descends toward the inguinal region, where it divides into a genital branch and a femoral branch. Medially, it courses near the external iliac vessels and the deep inguinal ring, while laterally it relates to the iliacus and the proximal thigh musculature. Superficial landmarks in this field include the inguinal ligament, the femoral triangle region with sartorius laterally and adductor longus medially, and the femoral nerve lying lateral to the femoral artery. Small, superficial trajectories of the femoral branch toward the anterior upper thigh contrast with the genital branch heading toward the spermatic cord. Anterior exposure matters because the genitofemoral nerve is a common source of postoperative groin pain and dysesthesia after inguinal hernia repair, orchiopexy, and approaches that place mesh or sutures near the deep inguinal ring. Clear separation from the ilioinguinal and iliohypogastric nerves is the teaching point, since symptom distribution can overlap, but the femoral branch typically supplies sensation over the upper anterior thigh, while the genital branch innervates the cremaster and scrotal skin. Nerve blocks and surgical neurectomy planning often hinge on these relationships to psoas major, the external iliac and femoral vessels, and the inguinal canal.
