Description
Centered in the bony orbit, the globe presents its anterior segment with a transparent cornea continuous posteriorly with the white sclera, framing a blue iris and central black pupil. Red extraocular muscles insert onto the sclera, their bellies positioned around the equator of the eyeball to control elevation, depression, adduction, and abduction. Posteriorly, the optic nerve emerges from the scleral coat at the posterior pole and courses proximally toward the optic canal. Fine episcleral vessels trace across the anterior sclera. Seeing the extraocular muscle insertions in relation to the sclera is useful when teaching ocular motility and the mechanical basis of strabismus, where underaction or overaction of a specific rectus or oblique produces predictable diplopia patterns. Surgical planning for recession and resection procedures depends on these insertion sites and their distances from the limbus, and the same anatomy underlies iatrogenic anterior segment ischemia when multiple rectus muscles are operated in one session. Optic nerve continuity at the posterior pole also anchors discussions of optic neuritis and the afferent limb of the pupillary light reflex. Small structures, big consequences.
