Description

Centered on the posterior pelvis, the sacrum and coccyx sit in the midline, flanked by the right and left ilia with their iliac crests sweeping laterally toward the posterior superior iliac spines. Inferior to the sacroiliac joints, the ischia descend to the ischial tuberosities, while the proximal femora align below, with the greater trochanters projecting laterally and the femoral shafts continuing distally. The vertebral column is continuous superiorly into the thoracic cage and scapular girdle, giving clear context for pelvic orientation in anatomical position. Posterior bony landmarks around the hip and pelvic girdle are the ones clinicians palpate, inject around, and use to triangulate pain. Sacroiliac joint dysfunction, sacral insufficiency fractures, and posterior pelvic ring injuries after high energy trauma all localize to the relationships between the sacrum, ilium, and ischium, and this posterior orientation makes those relationships easy to teach. It is also the cleanest way to reinforce how pelvic obliquity or leg length discrepancy can appear as asymmetry at the iliac crests and PSIS before you ever look at imaging. Landmarks matter. Use it for gross anatomy lab practicals and musculoskeletal blocks when students need a reference for palpation points, sacroiliac joint anatomy, and the transition from lumbosacral spine to hip. It also fits orthopaedic and radiology teaching materials that pair surface anatomy with AP pelvis and inlet outlet trauma series, or with CT reconstructions of the posterior pelvic ring.
