Description
Positioned in posterior anatomical view, the gluteus maximus is highlighted over the buttock, spanning from the posterior ilium adjacent to the posterior superior iliac spine down across the sacrum and coccyx, then sweeping inferolaterally toward the proximal femur. Its fibers converge to insert partly into the iliotibial tract along the lateral thigh and partly onto the gluteal tuberosity of the femur, just posterior to the greater trochanter. Superiorly, the thoracolumbar region and trapezius frame the upper back, while inferiorly the gluteal fold gives way to the posterior thigh where the hamstrings (biceps femoris laterally, semitendinosus and semimembranosus medially) run toward the knee. Bilateral symmetry is maintained for left and right sides. Clear landmarks. Gluteus maximus is the principal extensor of the hip from a flexed position, so this posterior relationship between its broad origin and its lateral insertion helps explain why it contributes both to powerful propulsion and to control of hip adduction via tension on the iliotibial tract. Inferior gluteal nerve injury, classically after posterior hip surgery or pelvic trauma, weakens stair climbing and rising from a seated position, producing a compensatory posterior trunk lurch at heel strike. The muscle’s bulk also defines the safe zone for intramuscular injection in the superolateral quadrant of the buttock, placed away from the sciatic nerve as it exits the greater sciatic foramen deep to gluteus maximus.
