Description
Posterior anatomy of a female trunk and lower limb is rendered with the gluteus maximus partially reflected to expose the fan-shaped gluteus medius on the lateral ilium. Its broad origin along the external surface of the ilium sits inferior to the iliac crest and lateral to the sacrum, while the muscle fibers converge distally toward the greater trochanter of the femur. Superiorly, the medius relates to the lower fibers of the latissimus dorsi and thoracolumbar fascia across the back, and inferiorly it blends into the contour leading to the proximal hamstrings on the posterior thigh. Layering is explicit. Gluteus medius matters because it is the primary frontal-plane stabilizer of the pelvis during single-leg stance, and weakness or denervation produces the classic Trendelenburg sign and contralateral pelvic drop during gait. Injury to the superior gluteal nerve (L4 to S1), classically during a posterior or anterolateral approach to total hip arthroplasty or after misplaced intramuscular injection, selectively compromises gluteus medius and minimus while sparing gluteus maximus. This posterior perspective also helps teach the safe superolateral quadrant for gluteal injections, keeping distance from the sciatic nerve and inferior gluteal vessels that course deep to the maximus.
