Description
Posterior trunk and lower limb musculature is presented in anatomical position, with the gluteus maximus highlighted over the posterior pelvis and proximal femur in an adult female. Its broad origin spans the posterior ilium, dorsal sacrum, and coccyx, then sweeps inferolaterally to insert into the iliotibial tract and the gluteal tuberosity of the femur, lying superficial to gluteus medius and the short external rotators. Superiorly, the trapezius and latissimus dorsi frame the thoracolumbar fascia and erector spinae; inferiorly, the gluteal fold transitions into the posterior thigh where the biceps femoris, semitendinosus, and semimembranosus descend toward the knee. Calf contours include gastrocnemius with the deeper soleus visible by form and fiber direction. Big muscle. Clear borders. Emphasis on gluteus maximus is clinically useful because it is the principal hip extensor in rising from a chair, stair ascent, and sprinting, and it stabilizes the pelvis through tensioning of the iliotibial tract over the lateral thigh. Denervation from inferior gluteal nerve injury (L5 to S2), seen in posterior hip trauma or iatrogenic injury during posterior surgical approaches, produces difficulty with forceful hip extension and reduced power on stairs. The posterior gluteal region is also a high stakes injection field: appreciating the superolateral quadrant relative to the greater trochanter and iliac crest helps avoid the sciatic nerve and superior gluteal neurovascular bundle that course deep to the gluteus maximus.
