Description
Posterior trunk and hip musculature are presented with the gluteus medius emphasized bilaterally on the lateral aspect of the ilium. From this posterior perspective, the muscle belly fans inferiorly and laterally toward its insertion on the lateral surface of the greater trochanter, sitting deep to where gluteus maximus would normally overlie it. Superiorly, the iliac crest forms the proximal boundary, while medially the erector spinae columns flank the lumbar spinous processes and descend toward the sacrum, placing the gluteal region in clear relation to the paraspinal mass. Latissimus dorsi, trapezius, deltoid, and the posterior arm contour (including triceps brachii) remain visible for orientation. A clear landmark. Gluteus medius is the primary abductor and a major pelvic stabilizer in single-leg stance, so this posterior-lateral depiction maps cleanly to Trendelenburg sign and gait disturbance after superior gluteal nerve injury. The course of that nerve across the deep surface of the muscle (between gluteus medius and minimus) matters in hip surgery and in lateral approaches to the greater trochanter, and it explains why hip abductor weakness can follow posterolateral exposure or retraction. Seeing the muscle’s broad origin on the outer ilium and its distal convergence at the trochanter helps anchor palpation and ultrasound-guided injection planning around the peritrochanteric space, where gluteus medius tendinopathy and trochanteric bursitis cluster clinically.
