Description
Shown in lateral profile, the thoracic vertebrae are aligned with stacked vertebral bodies anteriorly and overlapping spinous processes projecting posteriorly. Between adjacent spinous processes, the interspinous ligament occupies the interspinous spaces as a thin fibrous sheet running obliquely from the root of the superior spinous process to the upper border of the inferior spinous process. Along the posterior tips, the interspinous ligament blends with the supraspinous ligament, which continues as a longitudinal band superficial to the spinous processes. Intervertebral discs sit between vertebral bodies, and the adjacent paraspinal musculature is indicated lateral to the posterior elements. This ligamentous complex matters because it forms part of the posterior tension band that limits flexion of the thoracic spine and helps guide segmental motion. Tenderness between spinous processes after flexion injury raises concern for interspinous ligament sprain, and on MRI the interspinous and supraspinous ligaments are assessed in suspected posterior ligamentous complex disruption after thoracolumbar trauma. Surgeons also reference these midline ligaments during posterior exposure, and interspinous distraction devices or interspinous process decompression procedures depend on accurate appreciation of the interspinous space and the relationship of ligament to bone. A small structure, big consequences.
