Description
Centered within the pediatric thorax, the trachea descends in the midline from the laryngeal inlet to the carina, where it bifurcates into the right and left main bronchi that enter each lung at the hila. The right lung occupies the right pleural cavity and is divided into superior, middle, and inferior lobes by the horizontal and oblique fissures, while the left lung lies slightly more medial around the cardiac silhouette and separates into superior and inferior lobes by an oblique fissure. Superiorly, the apices project above the level of the first ribs, and inferiorly the bases rest on the diaphragm. Smaller bronchial branches course distally toward the peripheral parenchyma where alveoli form the gas exchange surface. For pediatric airway teaching, this arrangement matters because the right main bronchus is typically shorter, wider, and more vertical than the left, so aspirated foreign bodies and displaced secretions more often lodge on the right side, commonly in the right lower lobe when the child is upright. Lobation and fissure anatomy also guide auscultation and interpretation of lobar pneumonia patterns, as well as chest radiograph localization when a focal opacity respects a fissure.
