Description
Posterior neck and upper thoracic musculature is presented with the semispinalis emphasized beneath the more superficial splenius group. Superiorly, semispinalis capitis ascends to the occipital bone between the nuchal lines, lying deep to splenius capitis and lateral to the midline attachments of trapezius that would normally cover this region. Inferiorly, semispinalis cervicis is identifiable as a transversospinalis column running superomedially from upper thoracic transverse processes toward the spinous processes of the mid to upper cervical vertebrae, positioned medial to the splenius cervicis and closely applied to the posterior elements of the vertebrae. The occipital region, cervical vertebral alignment, and portions of the scapula provide fixed bony landmarks for reading these fiber directions. Seeing semispinalis as a distinct regional unit matters when you are teaching neck extension and contralateral rotation mechanics, because its long, oblique fascicles explain why deep posterior neck strain often localizes near the cervicothoracic junction and the upper cervical spinous processes. This posterior dissection-style view also supports clinical discussions of cervicogenic headache, where tenderness and trigger points in semispinalis capitis can refer pain to the occiput, and where interventions must respect the deep plane near the greater occipital nerve as it emerges inferior to the obliquus capitis inferior and pierces semispinalis capitis.
