Description
Arising from the right lateral aspect of the pontomedullary junction, the vestibulocochlear nerve (cranial nerve VIII) is shown emerging in close company with the facial nerve (cranial nerve VII) at the cerebellopontine angle. CN VIII courses laterally toward the internal acoustic meatus, positioned posterior and slightly inferior to CN VII, reflecting their shared transit through the petrous temporal bone. Neighboring rootlets from the trigeminal (CN V) superiorly and the glossopharyngeal, vagus, and accessory nerves (CN IX to XI) inferiorly help fix the brainstem level and the mediolateral spacing of the cranial nerve exits. Right-sided anatomy only. Clear landmarks. This lateral brainstem view matters because most clinically relevant CN VIII pathology is local to the cerebellopontine angle and internal auditory canal, where small changes in mass effect alter the relationship between CN VIII and CN VII. Vestibular schwannoma classically presents with progressive unilateral sensorineural hearing loss and tinnitus, then facial numbness or weakness as adjacent nerves become compressed, so the tight packing at the pontomedullary junction is not an academic detail. Surgeons approaching the internal acoustic canal via retrosigmoid or translabyrinthine routes think in exactly these spatial relationships, and trainees often confuse CN VIII with CN VII unless they can visualize their paired emergence. Neuroanatomy and head and neck courses use this plate to anchor cranial nerve numbering, exit zones, and the concept of the cerebellopontine angle as a surgical corridor. It also fits well in ENT and neurosurgery teaching files covering acoustic neuroma workup, brain MRI correlation, and facial nerve preservation strategies.
