Description
Arising at the pontomedullary junction, the vestibulocochlear nerve (cranial nerve VIII) emerges lateral to the facial nerve (CN VII) at the cerebellopontine angle, then courses anterolaterally toward the internal acoustic meatus. Proximally, its rootlets sit superior and lateral to the glossopharyngeal (CN IX) and vagus (CN X) nerves, which exit more inferiorly from the postolivary sulcus of the medulla. Nearby, the abducens nerve (CN VI) typically leaves the brainstem closer to the midline at the pontomedullary sulcus, helping orient the viewer to medial versus lateral relationships. Short, crowded real estate. Clinically, this configuration matters because the cerebellopontine angle is the classic site for vestibular schwannoma, where progressive unilateral sensorineural hearing loss and tinnitus often accompany imbalance as the tumor enlarges within or near the internal acoustic canal. The tight pairing of CN VIII with CN VII explains why facial weakness or decreased lacrimation can complicate larger lesions, and why the nervus intermedius and facial motor root deserve attention during microsurgical resection. The same anatomy underlies bedside localization when a patient presents with vertigo plus ipsilateral facial symptoms after posterior fossa pathology.
