Description
Running superior to inferior along the cervical visceral compartment, paired vagus nerves (cranial nerve X) descend within the carotid sheath lateral to the pharynx and larynx, giving off the superior laryngeal nerves that split into internal and external branches near the level of the hyoid. Medial to the carotid arteries and internal jugular veins, the laryngeal framework and adjacent pharyngeal musculature appear in red, with a midline airway column consistent with larynx and proximal trachea. Inferiorly, the recurrent laryngeal nerves course superiorly in the tracheoesophageal groove toward the cricothyroid joint, approaching the posterior aspect of the thyroid region before entering the larynx. Symmetry is emphasized. Despite the title’s reference to the vestibulocochlear nerve, the content aligns with laryngeal innervation, and that matters because the superior and recurrent laryngeal nerves define both voice and airway protection. Injury to the external branch of the superior laryngeal nerve during thyroidectomy can weaken the cricothyroid and blunt high-pitch phonation. Unilateral recurrent laryngeal nerve palsy, often from thyroid surgery, anterior cervical procedures, or mediastinal pathology (left-sided involvement with aortic arch disease), leaves the ipsilateral vocal fold immobile and raises aspiration risk. A small nerve, a large consequence.
