Description

Oriented in a posterior view, the heart is dominated by the diaphragmatic surface of the left ventricle with the right ventricle wrapping more anteriorly and only peeking around the inferior margin. The posterior interventricular sulcus runs inferiorly toward the apex, carrying the posterior interventricular artery (posterior descending artery) with an accompanying cardiac vein, while the coronary sinus courses transversely in the posterior atrioventricular groove between the left atrium and left ventricle. Superiorly, the base of the heart is formed by the left atrium, with the pulmonary veins entering posteriorly, and the great vessel orifices sit more anterior and superior, partly out of the direct line of sight. Posterior coronary anatomy matters because it is where coronary dominance declares itself: in right-dominant circulation the right coronary artery gives rise to the posterior interventricular artery, whereas a left-dominant pattern shifts this territory to the circumflex branch of the left coronary artery. This relationship is not academic when you are correlating an inferior wall myocardial infarction on ECG with angiographic findings, or planning a bypass graft to the PDA near the crux cordis where the atrioventricular nodal artery often originates.
