Description
Centered in the anterior thorax, the heart sits within the middle mediastinum, slightly left of the midsagittal plane, flanked laterally by the right and left lungs. From this front-facing perspective, the right atrium forms much of the right cardiac border while the right ventricle occupies most of the anterior (sternocostal) surface; the left ventricle contributes the left border and apex, which points inferolaterally to the left. Superiorly at the base, the ascending aorta and pulmonary trunk emerge, with the superior vena cava entering the right atrium on the patient’s right. Pulmonary veins course posteriorly into the left atrium and are implied rather than prominent from an anterior viewpoint. An anterior heart-in-thorax view matters because it matches how clinicians orient during surface anatomy, trauma assessment, and many open approaches to the mediastinum. The spatial relationship between sternum, lungs, and pericardial contents helps explain why penetrating injuries near the left fifth intercostal space at the midclavicular line threaten the left ventricle, while a midline approach risks the right ventricle and great vessels first.
