Description
Emerging from the left pulmonary artery at the hilum, the inferior lobar artery courses posteroinferiorly toward the lower lobe, typically passing in close relation to the left main bronchus and then dividing into segmental branches that accompany the bronchial tree. Proximal pulmonary veins return to the left atrium anterior and inferior to the arterial branches, while the aortic arch and descending thoracic aorta sit medial and posterior to the left lung root. Superiorly, the trachea with its cartilaginous rings bifurcates into the main bronchi, framing the central vascular pedicle. Clean relationships. This vascular pathway matters when planning or teaching left lower lobectomy and segmentectomy, where surgeons must distinguish the inferior lobar artery from adjacent venous tributaries to prevent hemorrhage or postoperative pulmonary venous congestion. The left lower lobe arterial anatomy also underpins interpretation of contrast-enhanced CT pulmonary angiography, since emboli and tumor-related encasement often declare themselves at lobar and segmental branching points rather than at the main pulmonary artery. Variants occur, including early segmental takeoff or shared trunks with lingular branches, and they change the operative sequence at the hilum.
