Description
Positioned retroperitoneally on either side of the vertebral column, the right and left kidneys sit in the posterior upper abdomen with their superior poles angled toward the inferior border of the rib cage and their inferior poles descending toward the upper lumbar levels. From a posterior orientation in a male, the kidneys appear lateral to the spinous processes and medial to the flank, with the renal hila oriented anteromedially even when the posterior surface is emphasized. Ureters emerge from the renal pelvis at the hilum and course inferiorly, hugging the psoas major region as they descend toward the pelvis. Upper and lower poles are clearly distinguished. That matters. A posterior view anchors surface anatomy and retroperitoneal relationships that clinicians rely on when localizing renal pain and planning access. Costovertebral angle tenderness, pyelonephritis, and obstructing ureterolithiasis are often first framed from the back, and this orientation clarifies why renal pathology can refer pain to the flank and why percussion is performed below the 12th rib. For intervention, the posterior approach underpins percutaneous nephrolithotomy and nephrostomy tract planning, where the goal is to reach a posterior calyx while avoiding pleura, colon, and segmental renal vessels that branch anterior to posterior within the renal sinus.
