Description
Seen in lateral profile, the male L4 vertebra presents a broad vertebral body anteriorly with a short, stout pedicle extending posteriorly to the vertebral arch. The laminae converge toward the spinous process posteriorly, while the transverse process projects laterally from the junction of pedicle and lamina. Superior and inferior articular processes define the zygapophyseal (facet) joints above and below, and the superior and inferior vertebral notches contribute to the intervertebral foramen adjacent to the L4 to L5 segment. L4 is a practical landmark because it sits near the biomechanical transition into the lumbosacral junction, where axial load and shear increase, and where degenerative disc disease and spondylolisthesis commonly declare themselves. A lateral view clarifies why pars interarticularis defects matter, the pars lies between the superior and inferior articular processes and becomes the weak link in isthmic spondylolisthesis, classically at L5 but assessed in relation to L4 to L5 alignment. It also supports procedural orientation: the intercristal line (Tuffier’s line) typically crosses the L4 spinous process or L4 to L5 interspace, a routine reference for lumbar puncture and neuraxial anesthesia.
