Description

Oriented to the lateral aspect of the right knee, the patella sits anterior to the distal femur within the extensor mechanism, with the lateral patellofemoral ligament spanning from the lateral margin of the patella toward the lateral femoral epicondylar region and adjacent capsule. Deep to it, the patellofemoral articular surfaces align as the patella tracks within the trochlear groove, while the tibial plateau lies inferior to the femoral condyles at the tibiofemoral joint line. Laterally, the fibrous capsule blends with the iliotibial tract superficially and approaches the region of the fibular head where the fibular (lateral) collateral ligament typically courses. Small but specific. Lateral patellar restraints matter when you are teaching or treating instability patterns that are not dominated by the medial patellofemoral ligament. After a lateral patellar dislocation, attention often stays on MPFL injury, but iatrogenic lateral tightness after aggressive lateral retinacular release or scarring can also drive medial patellar subluxation and anterior knee pain.
