Oriented in a true lateral perspective, the knee is expected to be visualized with the patella positioned anterior to the distal femur and the proximal tibia extending inferiorly, with the lateral patellotibial ligament (a lateral patellar retinacular thickening) coursing obliquely from the lateral border of the patella toward the anterolateral tibia near the tubercle region. Along its superficial plane you would logically expect to appreciate adjacent lateral retinacular fibers and the iliotibial tract overlying the lateral femoral condyle, while deeper landmarks include the lateral femoral condyle and the tibial plateau. Relationships are clear in this projection: patella anterior, femur superior, tibia inferior, and the ligament spanning anterior to the lateral tibiofemoral joint line. Clean bony landmarks anchor the soft tissue. Clinical relevance centers on patellar tracking and lateral restraint. Tightening, scarring, or iatrogenic over-release of the lateral retinacular complex, including the lateral patellotibial ligament, can shift the patella medially or laterally and alter contact pressures at the lateral patellar facet, a common contributor to anterior knee pain and postoperative dissatisfaction after lateral release. This lateral view also helps teach why anterolateral tibial landmarks matter during reconstructive work around the extensor mechanism and why lateral retinacular tenderness can coexist with normal cruciate and collateral ligament exams.
