Description
Seen in lateral profile, the patellar ligament (patellar tendon) courses inferiorly from the apex of the patella to its broad insertion on the tibial tuberosity, lying anterior to the tibial plateau and just medial to the fibular head. The distal femur and proximal tibia frame the joint line, with the fibula partially visible posterolateral to the tibia. Along the lateral aspect, the iliotibial tract runs superficially toward Gerdy’s tubercle, and the fibular (lateral) collateral ligament spans from the lateral femoral epicondyle to the fibular head, posterior to the patellar ligament. A lateral view clarifies the extensor mechanism’s anterior tension band and its relationship to common surgical and sports medicine landmarks on the proximal tibia. Patellar tendinopathy (jumper’s knee) most often localizes at the proximal patellar ligament near the inferior pole of the patella, while Osgood-Schlatter disease affects the tibial tuberosity apophysis at the distal attachment in adolescents. Alignment matters. This perspective also supports planning and teaching of midline anterior approaches for tibial tubercle osteotomy, where the patellar ligament insertion guides both osteotomy placement and protection of the anterior tibial cortex.
