Description
Emerging at the porta hepatis, the left hepatic artery courses superiorly and to the left toward the left lobe, then breaks into smaller intrahepatic branches that run alongside the portal venous and biliary conduits within the portal triads. From a posterior perspective, the vessel is read against the visceral surface of the liver near the fissure for the ligamentum venosum, with the caudate lobe lying posterior and medial and the left lobe extending anterior and lateral. Semi-transparency of the hepatic parenchyma permits the arterial arborization to be followed distally as arterioles spread toward the subcapsular regions. Clean orientation. Posterior visualization of the left hepatic artery matters when you are planning hilar dissection, since arterial variants are common and small caliber branches can be injured while mobilizing the left lobe or working near the venous ligament. In left hepatectomy, split-liver transplantation, or biliary reconstruction, identifying the left hepatic artery in relation to the left portal vein and left hepatic duct helps prevent ischemic injury to the bile ducts, a known driver of anastomotic stricture and biliary leak. This angle also supports teaching of segmental perfusion patterns relevant to Couinaud segments II, III, and IV. Suitable for hepatobiliary surgery atlases, transplant conference slides, and anatomy teaching on the celiac trunk, proper hepatic artery, and their intrahepatic branching in a male liver. It also fits radiology-pathology correlation material when discussing arterial-phase enhancement on CT or MR angiography and when mapping vessels before transarterial chemoembolization.
