Description
Posterior anatomy of the male upper abdomen is presented with an overlay that localizes the liver (hepar) beneath the right hemidiaphragm and inferior to the posterior thoracic cage. The right lobe (lobus hepatis dexter) occupies the right upper quadrant lateral to the vertebral column, while the left lobe extends medially toward the epigastrium. Along the posterosuperior surface, the region of the bare area and the course of the inferior vena cava with the hepatic veins are implied deep to the parenchyma, and loops of small and large intestine appear faintly inferior to the liver’s inferior border. The malignant hepatic mass is emphasized by irregular contour and focal discoloration against surrounding parenchyma. Posterior orientation matters when you teach or plan around the liver segments that abut the retrohepatic inferior vena cava, because lesions in the posterior right lobe (segments VI and VII) behave differently surgically and on cross sectional imaging than anterior or left lobe tumors. A mass in this location can distort hepatic venous drainage and compress the caval groove, which helps explain Budd-Chiari type outflow obstruction or lower extremity edema in advanced disease. It also mirrors what radiologists describe on CT or MRI when characterizing hepatocellular carcinoma versus metastatic deposits by capsule, contour, and adjacent vascular involvement. Bleeding risk is the point. Use this artwork in gross anatomy and hepatobiliary teaching to reinforce where the liver sits relative to the posterior thoracoabdominal wall, and in oncology or surgical texts discussing posterior segmentectomy, right hepatectomy planning, or retrohepatic IVC exposure.
