Description
Originating from the popliteal artery just distal to the knee joint, the anterior tibial artery passes anteriorly through the proximal opening in the interosseous membrane to enter the anterior compartment of the leg. From there it descends on the anterior surface of the interosseous membrane, accompanied by the deep fibular (peroneal) nerve, running between tibialis anterior medially and extensor digitorum longus laterally, then continuing toward the ankle as the dorsalis pedis artery. Proximally, the femoral artery is seen in the thigh giving rise to the popliteal artery, while venous return is represented by paired deep veins and the more superficial great saphenous vein ascending on the medial side of the leg and thigh. Emphasis on the initial segment of the anterior tibial artery makes this view practical for correlating compartment anatomy with vascular access and injury patterns. Penetrating trauma or proximal tibial fractures can compromise the vessel as it traverses the interosseous membrane, and its course is central to understanding anterior compartment ischemia in acute compartment syndrome. This pathway also frames surgical and endovascular planning for tibial revascularization in chronic limb threatening ischemia, where patency of the anterior tibial artery and dorsalis pedis inflow drives distal perfusion.
