Description
Arising within the dura mater of the middle cranial fossa, the middle meningeal vein accompanies the middle meningeal artery along the internal surface of the temporal and parietal bones, running superiorly and posteriorly toward the pterion region. Its main trunk courses lateral to the cerebral hemisphere, external to the arachnoid and pia, and converges toward the skull base to exit at or near the foramen spinosum, adjacent to the mandibular division of the trigeminal nerve (V3) as it traverses the infratemporal region. A lateral cranial perspective logically also includes the cerebral cortex in profile, the brainstem and upper spinal cord, and contrasting arterial and venous channels mapped over the cranial cavity. Orientation is clear. The vessel is seen as an extracerebral venous pathway rather than a cortical vein. Clinically, this vein sits in the same anatomic corridor as the middle meningeal artery and is relevant when teaching epidural hemorrhage patterns after lateral skull trauma near the pterion, where meningeal vessels can be torn against the calvaria. Its proximity to the foramen spinosum matters in skull base approaches and in interventional planning, since meningeal venous drainage communicates variably with the pterygoid venous plexus and can participate in dural arteriovenous fistula pathways. Surgical dissection around V3 and the infratemporal fossa benefits from a clear mental map of this venous companion vessel. Small structure, big consequence.
