Description
Sweeping around the left orbital aperture, the orbicularis oculi is rendered as a continuous sphincter with its orbital portion encircling the bony rim and its palpebral portion thickening within the upper and lower eyelids. Medially, the muscle fibers converge toward the medial palpebral ligament and lacrimal region, then fan laterally toward the lateral palpebral raphe near the zygomatic bone. Deep landmarks from the skull model remain readable, including the frontal bone superiorly, the maxilla and zygomatic bone anterior and lateral to the orbit, and the nasal cartilages positioned anteromedially. Cervical vertebrae sit inferior to the occipital bone, anchoring the profile orientation. A left oblique, near-profile view like this is where the orbital and palpebral parts stop being abstract terms and become a surgical map, since the muscle’s thickness, fiber direction, and relationship to the eyelid margins determine eyelid closure, blink mechanics, and the spread of periorbital edema. Blepharoplasty planning and repair of eyelid lacerations depend on respecting the preseptal and pretarsal orbicularis planes to avoid postoperative lagophthalmos or lower-lid malposition.
